Vaccines will be available on April-4th for people who qualify under Phase 1A. You must register using the form below. No Walk-ins allowed.

  • Temple Hours: Weekdays 9:30 a.m to 12:30 p.m & 5:00 p.m to 8:30 p.m. Weekends: 9:00 a.m to 8:30 p.m.
